steps to reproduce the issue:
1. add some header 1 and header 2 
2. Insert -> "Indexes and Tables" -> 
3. save file in .doc format and re-open it
4. Then the TOC content will lose and TOC -> "Outline" was unchecked by OO
5. then close and re-open the file 
6. TOC lost
