It's important to remember that I'm trying to create a simple form which does not rely on a DB... in fact, the intent was to just make it available and let an agent fill it in and print directly. So it won't hurt my feelings if you tell me this is the wrong approach as long as there is a way to do this somewhere in OOo... :) Even a non-printing "settings selection" page acting as the "DB"... There is just no need to go through the setup, support and training of an actual DB when all I want is to provide a simple fill in the blanks form....
