I've closed and reopened the document, and the orphaned Dialog disappears, 
which is consistent with the behavior of restarting OO.

I would suggest that the orphaned dialogs be visible at all times (else the 
name conflict makes no sense), or create an empty macro to go with the dialog 
if it comes first. This seemed natural to me, since I could edit a dialog 
without much of a learning curve, whereas starting the macro first would have 
been a bigger task.
