I tested on Win XP Pro SP2. Problem is very easy to reproduce (five clicks): Click on Use Wizard to Create Form... A OpenOffice.org Writer window appears with the Form Wizard window on top. Step 1 - click on >> and Next Step 2 - click on Next Step 5 - under Arrangement of the main form select Columnar - Labels Left Now the Form Wizard window is gone. It reappears after you minimize and maximize the window by using the button on the taskbar. After that you can finish the wizard without further problems.
