A) Steps to take
1. Add alphabetical index
2. Use headings with numbering as base for creating index entries.

B) What happens
Out of unexplainable reasons some entries are shown with the chapter numbering
in the index. Sample document will be attached. Look in index for entries under 
"F".

C) What should happen
Should ignore chapter numbers when entry was created from heading.
