This is done in the Index dialog on the "Styles" tab. Please for the future, ask such questions in one of the OOo newsgroups or mailing lists as long as this is an issue tracker and not a support forum. Regarding your notes problem, I do not understand your description. What kind of notes are you talking about? And where do you try to put headers there?
