2009 -------
I've openned a .doc document and saved it in .otd.
Then I decided to add a Table of Content.

Even before I created any entry, the table of content display 4 entries:
Risque
.Probabilité
..Conséquences
Faibles

... I tried to located thoses entries, but they just don't exist. (Yes, Field
Shadding is enabled)

So I started to add my own entries, even if I had ghosts entries.
Then I was able to access the "Edit Index Entry" dialog.

The Edit Index Entry dialog doesn't show the ghosts entries.

That mean that the code to generate the table of content and the code to browse
the entries is different.

Also, I created a ghost entry by myself. I added an entry, then deleted it, then
recreated it. I now have 2 entries in my table of content. (Yes, I updated my
table of content)

Also, a little suggestion:
What would be use full if not a "Microsoft Access like" dialog box (that
obviously don't work too well), but a listing of all the entry in a multiline
select box. So we could easilly view all the entries at once...
