Since it is only reproducible on some machines, it could be that it is a timing problem. At least the binfilter project seems to contain no changes from OOo3.1 that could cause the problem. One of possible reasons for the problem could be that a new unknown entity is provided to the binfilter through flat-xml filter, and the unexpected header is generated because of this.
