1) It was relatively easy to replicate this defect using "Pre.ppt" attached by 
imccowatt.
2) At the same time I found that the cursor’s position was important. When 
cursor was in the first line of the Master Slide, the “Bullets and Numbering” 
dialog box has the appearance on "MasterSetup-CursorOnFirstLine.JPG" and I was 
not able to reproduce this defect. Please pay attention to the very top “bar” 
corresponding to “level 1” in the right part of this dialog box – it’s aligned 
normally.
3) When cursor was in any line of the Master Slide except the first one, 
the “Bullets and Numbering” dialog box has different appearance and I was able 
to reproduce this defect. Please pay attention to the very top “bar” 
corresponding to “level 1” in the right part of this dialog box. It was already 
moved to the right "MasterSetup-CursorOnAnotherLine.JPG".
4) It’s a well known fact that customization of “Bullets and Numbering” in MS 
Word is one of the largest design errors in MS Office history. Probably 
only “Mail Merge” feature can compete for the 1-st place with it. I do not know 
how much MS code and design ideas are shared between MS Word and MS PowerPoint. 
So I can not exclude a possibility that the same defective design is used in MS 
PowerPoint. At least visually it reminded me of my unsuccessful battles with 
Bullets and Numbering customization in MS Word. It means that we may have a 
situation when this Impress feature works OK but is forced to deal with a 
defective MS PowerPoint document.
