I just FIXED this problem on my own system by upgrading the MyODBC driver from
3.51.05 to 3.51.11. Here is what I was typing before deciding to try updating
this driver:

I can confirm this behavior on WinXP SP2 using OOo 1.9.79 snapshot and a MySQL
backend database (MySQL version 4.0.21 running on Windows 2000 SP4). MyODBC
version is 3.51.05. Here are the MyODBC settings obtained from odbcad32.exe and
double-clicking on the established System Driver that connects to our database.
This configuration works fine on MS Access 2000 and other programs:

Port: 3306 (default)
SQL Command on Connect: <blank>
Only options checked are:
- Don't optimize column width
- Return matching rows
- Use manager cursors

In the Query Designer, when clicking "Add Table", I only get a '*' in the table
dialog for any table that I place in the design viewer. It does successfully
list all of the tables, though.

Other similar places that don't work properly:
- Use Wizard to create Query - You can select the table, but the fields never
fill in the "Available Fields" section, forcing you to cancel the wizard.
- Form Designer - I add a new form, set the Data > Content to a MySQL table or
query, then click "Add Field", and no fields appear in that dialog either.

If I type the SQL query manually in the Query Designer, it does return the
proper fields, so I know we have a live connection to the data, it just doesn't
update properly in some places.
