I tested your database with 3.2, the issue happens, error message came up.
When I tested with OOO320m15, the development version to OOo 3.2.1, the form
work correctly.
Several issues with forms solved in m15 development version, the reason to set
this issue as WORKSFORME.
If you have a problem with this version open issue back.
