Is incredible that there is no way to remove all index entries in a single
operation!
When I use MS Word:
With the XE fields displayed, use the replace function 
to replace ^d XE with nothing or ^19 with nothing.

Proposal (Without changing the UI)

Delete all similar entries =  CTRL + ALT + click button [Delete]
Delete all entries         =  CTRL + ALT + SHIFT + click button [Delete]
