Back to the original problem: If you create a new document, extension supplied
menu items and toolbars are visible, but not in documents that were already open
before installing the extension. If you close and reopen the "old" document,
then you see it.
Proposed solution was to do an office restart to work around the actual bug. Why
this? We should do as always and fix the actual problem.
If we see demand for restart-after-extension-installation feature, we should
create a separate issue for this, not highjacking a bug report for a related,
but actually different problem.
