I use latest OpenOffice:
OpenOffice.org 3.2.1 
OOO320m19 (Build:9505)
ooo-build 3.2.1.4

The problem is actually from the other side. When I create an autofilter
(Data->Filter->AutoFilter) there are entries like "- empty -" and "- not empty
-" in the drop down list. You can use the attachment empty_cells.ods for
reference. You see that column B contains cells with values "", A, B and real
empty. When option "- empty -" is selected then only "real_empty" row is shown,
but when "- not empty -" is selected, then all rows are shown except real_empty.

Your proposed solution to use empty item from drop down list works partially for
me. I see this empty item in a list like a horizontal line, which I CAN NOT
select with a mouse, but only with arrow keys. And it works - all empty cells
(both real and with "") are shown.  But still, there is no possibility to see
not-empty cells (where string length is more than 0).

I understand the difference between blank and empty, but is real situations
empty string and no string means the same. When user just looking at the cell -
it looks empty in both cases.

Bottom lime: My suggestion is to modify autofilter in order to make no
distinction between blank and empty. As a result, "- not empty -" item will work
more usefully for human beings ;) 

BTW: MS Excel makes no distinction in this case. I actually was using the same
document in Excel and OO and faced troubles in OO because I can not use
autofilter to select team members which have NO task assigned (cell is empty
because formula writes "" there). This is real world example...
