I now know this happens sometimes after a big merge is being interrupted by killing OOo. This will leave the lock file /tmp/.~lock.SwMM0.odt#. Such a lock file prevents new instances from running the mail merge. I solved it by preventing that OOo is killed by doing a mail merge (this was a bug in one my own scripts), and by giving each OOo instance it's own tmpdir.
