I used a mixture of paragraph styles in an attempt to get an outline working. At
one point I typed a list of headings and later tried to convert them to actual
paragraph headings. I used numbered list, outline numbering, format paragraph,
format bullets and numbering. Somehow the paragraph headings have taken on
styles that prevent the paragraph numbering from functioning. These hidden
styles can't be removed with the usual methods (Format/Default formatting or
'Clear formatting' from the toolbar).
I have unzipped the document and analysed the content.xml file, around the
headings that don't number properly there are extra xml like this:

I could not find a way to view this from OO, or to edit it. The only clue is
that the paragraph numbering does not appear as expected.
