regina, thanks for the excellent description. Unfortunately, following it
step-by-step, I still don't encounter the problem :(
There must still be a subtle difference in what we both are doing.

Your attached database is in fact broken. It could easily be repaired, and we
could even include a fix in OOo to repair it automatically. However, this
wouldn't address the root cause :(

Looking at the file, it seems to be broken from the very beginning. Thus, I
assume the mentioned subtle difference is somehwere in how/where you initially
created the database, or what you do between you created it and connected the
first time, or something like this.
