Unfortunately, following it step-by-step, I still don't encounter the problem :( There must still be a subtle difference in what we both are doing. Your attached database is in fact broken. It could easily be repaired, and we could even include a fix in OOo to repair it automatically. However, this wouldn't address the root cause :( Looking at the file, it seems to be broken from the very beginning.
