2005 -------
Open a document, choose Extras > Chapter-numbering. Enter some settings for 
the numbering. At the bottom there is a button "Form" (in the German OO 
version). Click on it and choose "saves as", save the numbering as test. 
 
Open a new document, choose Extras > Chapter-numbering, click on "Form". You 
can see the saved entry "test". 
 
Close all OO and re-start it. Open a new document, choose Extras > 
chapter-numbering, click on "Form", notice that the entry "test" is gone. 
 
A side-note. Why is the button lable "Form"? I would suggest something like: 
"Choose/Save as template"
