Only used alsamixer to do it so far (although amixer seems to agree that the appropriate channels are unmuted). Not sure where to proceed next (although I'll include the obligatory outputs of lsmod etc after this), but I'm beginning to wonder if this is just the latest Santa Cruz problem. I've posted to this list before about the gyrations I've had to go through to get some channels to produce sound (e.g. to get sound out of the Line input: mute the CD input, which will also mute the PCM input). Yeah, I know, weird, but I begin to recall that I'd been playing with Santa Cruz settings under Windows a week or so ago, and that may have started the problem.
