As far as I can tell, this shows that your laptop has both an analogue audio output (the normal 3.5mm jack socket) on card 0 device 0 and an IEC958 digital (SPDIF) output on card 0 device 4. Depending on the hardware, the SPDIF output might be a separate connector (probably an optical jack) or it might be integrated with the analogue one, either as a switchable analogue/digital output selected through a mixer setting (probably) or a combined analogue/optical jack (if you can see a red light in the headphone socket then it's one of these). /proc/asound/cards only shows one card because there is only one sound card present - it just has two output devices on it which is what are listed by aplay -l.
