There are several report forms in .ps style in the example 
directory.  Move the one you want to your /config/ directory, and 
then add it to your tapeype entry in your amanda.conf someplace 
between the curly braces like this:

   lbl-templ "/usr/local/etc/amanda/DailySet1/DDS2.ps"

That particular one makes a very small report that can be cut out 
and pasted/taped/inserted to a DDS-2 shell, however I quit that 
because of the possibility of contaminating the tape with the paper 
debris.  In your case I'd use the DIN-A4.ps file and punch it for a  
binder.
