It means that Amanda got some sort of error while trying to re-read
the label it had just written to a tape.  The (volume may be erased)
portion indicates that most likely something was written to this
volume, so it's likely to be effectively erased.

I'll leave the remaining, good questions to others on the list.  It
might be helpful to post some information about the hardware you're
using - the really expensive stuff can get you an extra 9 or two of
reliability.
