Yes, I am filtering from local.  I put that there so I could do filter
testing from a local account.  Once I took it out, things worked
normally with $max_servers=1.

Perhaps the readme should mention that the forked version of
$notify_method should still be used if you are doing local filtering.
In my case, it was just there for testing, but I'm sure there are some
people who have a legitimate need for it.

It looks like this fixes my problem.  Will this change to the Courier
patch be included with the next Amavis release?
