Which version of amavisd-new?
Show the log, increase the log level if necessary (at least 2 or 3).

The $final_bad_header_destiny=D_PASS does not prevent quarantining, it
only controls forwarding; quarantining has its own independent controls.
Are you sure the message was not delivered? Perhaps it was blocked
for some other reason (spam, virus, rejected by MTA, ...).
