I don't know why this would happen. One thing you might test is to
bypass banned_header tests in addition to passing them.

@bypass_banned_checks_maps = (1);

If you don't have any luck, then the next step would be send a test
message through (a message with a bad header) and log the transaction
at $log_level = 5; Then study the results for clues. It may be helpful
if you state what version of amavisd-new you are working with. Rather
than setting $log_level = 5; you might consider using
@debug_sender_maps:

# If sender matches ACL, turn debugging fully up, just for this one message
@debug_sender_maps = ( ["[EMAIL PROTECTED]"] );
