Only at amavisd startup time: when code in amavisd.conf executes
a call to read_hash. A lookup table contents is then loaded in memory
and a file is closed.

Dynamic lookups are available only through SQL or LDAP.
For simple SQL lookups a file-based SQLite solution is just fine.
