Peter, 
Assuming full duplex and no rf switching requirement (aka recieve only) my 
particular choice is DEMI's series of preamps. Cost $75 built and tested. If 
you're like me and don't mind a little solder smoke, and have access to test 
equipment for alignment, the kit is available for 45$.

PS: I use mine with a pair of quadrifiliar helix antennas.
