It can be reproduced. 1. In the visual editor, drag an image from the left hand side, to anywhere on the screen 2. Drag anything else and place it under the image (for example, some text) 3. Go to the XML code and try to edit the item you dragged below the image 4. It will mess up the code and you wont be able to undo it because Ctr-z doesn't work
