Appears that your MMC is in bad shape.

Try reformatting the card.  If you still get the errors, try a
different card.  If you are using Linux, I found that using a Windows
box to format it initially worked for me, then used 'fdisk' on my
Linux machine to format again as mmcblk0p1=fat and mmcblk0p2=ext3.
