I have absolutely no explanation for what is going on here ...

I opened your model and saw the same problem after generation, I tried
several things like removing/reassigning the controller but nothing worked

so what I then tried was to remove your CSS2004-MDA model from the UML
model and tried a minimal sample in a new package, and it worked

then I tried moving all of that into the (now empty) CSS2004-MDA model,
and it failed again

I tried renaming the model to something else and it failed again

lastly I created my own model (when I say model it is the little folder
with a triangle inside - see inheritance tree) and things worked again,
when I renamed that model to CSS2004-MDA it worked too (I was thinking
maybe not all names are well read from XMI or something)

I don't know what happened with your model because the XMI looks fine to
me, so my advice: create a new model in your UML model, call it 'wouter',
drag your package tree inside that new model, delete the old model and
rename 'wouter' to 'CSS2004-MDA' ... that should work (it worked here,
just tried it again)

ps: you had some other minor modeling errors but they were easily solved,
andromda printed validation messages for them so it should not be too hard
