Have a look at phpESP: http://sourceforge.net/projects/phpesp/ A minimal installation consists of Apache, PHP 4 and MySQL. Once the system runs, a web-based editor offers more than just the usual 1-question-poll. Although the feature is not documented, you can add HTML (i.e., also <img>) to a poll.
