For obtaining disks, this list, as well as the Swap List, are very good choices :-) .

The Apple II series use double density 5.25" disks. High density ones *will not* work. The Apple II can use both single sided and double sided disks will suit you, but since the Disk II is a one-side disk drive, to use the second side you will have to cut a second "read-write" notch. To make it, you will ahve to place it at the same place of the one made by the manufacturer, but on the opposite side, so when you "flip" the disk, the new notch stays at the same position as the original one was. Double sided, double density 5.25" disks are often referred to as 360 Kb diskettes.

About 3.5" disks, almost all Apple II computers will need double sided, double density disks. No other kind will work. But, if you have an Apple SuperDrive with its SuperDrive controller card (a setup that isn't very frequent on an Apple II), you can also use double sided, high density 3.5" disks. Single sided 3.5" disks (the ones used by the original Macintosh) will not work in any way. Double sided, double density disks are sold as 720 Kb or 800 Kb disks, where high density 3.5" disks are the standard 1.44 Mb disks used by current PCs.
