Meenakshi, Open the form in the Admin tool.. Go to Form -> Current View -> Fields in View
The fields that seem to have disappeard from your form must be sitting in Fields Not In View... You can bring them back in view again from here... No the field listing thorugh admin tool didn't show that fields. But the active links run on that form listing all the 50 fields in qualification field and all in the actions.
