I had raised this as a ticket to Remedy Support and they recognized it as a
software bug. I do not recall the bug number. I tried to find that E-mail
but can't seem to locate it.

An interesting thing I had found about a particular ticket that had NULL
entry ID's in the email notification was the result of this query.. Incident
with Entry ID INC000000000013 had a NULL Web_Incident_ID, while it did get
an Incident Number INC000000000059. I didn't get a chance to get to the
bottom of this problem, but it could be that the Web_Incident_ID is pushed
to the notification form to send out the notification resulting in a null
eid.

Mid-Tier URL for the case.  When I do, the URL has "eid=NULL" at the end of
instead of "eid=<Request ID>".  The Request ID shows in the text I send in
the notification, but does not carry over properly to the URL.  Any ideas?
