Does anyone else know if BMC publishes a comprehensive forms/fields 
definitions for  ITSM?

Yes, BMC has one for download, although it is not an exhaustive
description of every form and field, it is still important for
understanding some of the conceptual data relationships.
 
On the BMC support site, under Product Documentation, then for the BMC
Remedy Service Desk 7.0.3 application, look for the document titled:
 
(White Paper) BMC Remedy IT Service Management 7.0 Architecture
 
If you want to understand CMDB classes and relationships, then you will
probably want to look at the HTML documentation that ships with the CMDB
product itself.
