Once I run the script it created that view on the SQL side now how do you
create an ARS view form from this? Or how can I make that show up on the
Remedy side like the admin tool or user tool?

If you are using a version of SQL Server you could use the attached script to
create a SQL View, then create an ARS View form for the SQL View. Here is how
my screen looks:
