So although the SYS:Action table has workflow to delete the processed records that are no longer needed on that table using as in that example, Application-Delete-Entry "SYS:Action" 000000000407386, it has no way to lower the high water mark caused by the continuously growing table.. Over time that table is likely to occupy a lot of disk space although the number of entries in the table may be quite low.
I’m guessing that may be one of the problem. Ask your DBA to check into that and see if that’s the problem’..
