We are in itsm 7.6.03 p002 and we are also noticing this. We saw this happening when when user tries to open a dialog on click of a menu or button. This does not happen to all the users all the time. Only one or two users notice it while it works fine for all others. As you mentioned the workaround was to flush the midtier cache or sometimes user's browser cache. Do you have a defectID that BMC created for this issue?
