Dan, You probably have an error showing up in the Tomcat logs that can direct you to the problem. They should be in the Apache Tomcat install directory in the "logs" folder. Open up the logs that have "stderr" and "stdout" in their names. You may need to turn on the logging to a more detailed level, but by default any errors should show up.
Also, the Mid Tier does install Tomcat for you, so it may be easier for you to wipe out your Tomcat install and just let the Mid Tier installer do it. I've performed the install that way with no problems, and I've also installed it the way you have and manually deployed the WAR file with no problem. However, it's likely that you need to make some changes to Tomcat to make it more secure and run more efficiently even after you fix the issue you are running into.
