What do your approval process configuration records look like - is there one 
that begins with PIP and moves to SFA upon approval?

We have an approval phase at SFR and SFA.  If I create a change with no ad-hoc 
approvers, the change moves to SFR.  I approve at SFR and the change moves to 
SFA.  This is expected behaviour.  If I create a change and add an ad-hoc 
approver at PIP and approve it at PIP, my change automatically goes to SFA and 
bypasses SFR.  I dont even hit next stage when I am in PIP. 
  
It seems that somehow the system thinks my ad-hoc approval record is the last 
approval record in the SFR approval phase.  Has anyone else had this occur or 
have any suggestions on what to look for?  I have scrounged through the 
approval logs and the filter logs for two days and I have been unable to 
pinpoint the issue. 
  
Any help is appreciated.
