Why not have Process Designer create the task directly? Any tasks in the process will be created as TMS:Tasks anyway when the process is running for an Incident. You can have the process use a condition to decide if the task is assigned or not and you should not need to create the supporting data such as TMS:Flow, etc. Regarding your first task running automatically, is the value on the Incident? If so, you can use a Set Outcome action to automate it.
