The Work Order is included with the SRM application and is tied to a Service Request. If you want to create a standalone Work Order using a WEB Service you will need to create the form you suggest with the required fields and any other fields you will need and the workflow to push to the OOTB Work Order similar to the way the Incident Create form is used by a WEB Service.
