It will be difficult to answer that without knowing a little more about your environment and actually doing some poking around but since it seems to be a change to a user profile, I would go with checking some of the integrations which either add or modify user accounts, which might have probably caused that updated even if was not supposed to, because of some defective coding or wrong data supplied from another external system.
The other possible explanation is someone actually did it either intentionally or accidentally, and it might have been possible to know that if you had not already reverted the user back by checking the last modified by username - perhaps too late for that now unless you have some sort of an auditing process in place. The only difference was that the one shown...the current button was selected and for the other the disabled button was selected. This user was in the list in the recent past so I guess marked current. I changed it to current and he shows up now.
