Hey Tammy this is pretty simple to do. Create a new .aspx page and name it what ever you like.
Now put a form on the paoe, then put form fields on the page that match the columns in your database. Example email would be called email. You can do this in the property inspector. Make sure to put a smbmit button and reset button on the page in the form. When your done go to server behaviors and choose insert into database then fallow the prompts form there that you are asked.
