Hi Dean, thanks for your suggestions on this one and my other recent
questions.

The solution to this problem wound up being simple and somewhat
obscure:

For anyone that experiences similar problems, seee this:
http://distribucon.com/blog/archive/2005/08/16/684.aspx
And this: http://www.eventid.net/display.asp?eventid=1000

The short version: go into IIS and make sure the app has the "Cache
ISAPI Application" checkbox checked as well as the default web site
in IIS . I know i've unchecked this box before but can't remember
doing it recently.

I had a bunch of these in my event log:
Event Type: Error
Event Source: Application Error
Event Category: (100)
Event ID: 1000
Date: 11/4/2005
Time: 5:37:08 PM
User: N/A
Computer: CMOHAN
Description:
Faulting application inetinfo.exe, version 5.1.2600.2180, faulting
module aspnet_isapi.dll, version 1.1.4322.2032, fault address
0x0001dfee.
