you should set $denySMTPLog, then you can see the logging of 
>denySMTPConnectionsAlways
.

There are much more exceptions (whitelists, etc) for 
>denySMTPConnections

But 
>denySMTPConnections
 and 
>denySMTPConnectionsAlways
 are really meant for manual entries.
