I've lost this email:
removing /root/assp/spam/6705.eml  [email protected]' is in 
Whitelist

I've since found out that the email was sent on behalf of this user by 
another entity and did not originate from his mail server.  I think that 
was the problem.

I'll post headers if I can locate another example.
