Well I can't answer your whole question - but I do know that some Swiss
ISPs
(most notably Swisscom) have started in spring 2010 to redirect all
connections over Port 25
from their DSL networks to a central mail gateway where every
non-authenticated traffic gets
blocked. This is enabled by default and the ISP can disable the filter
when the customer kindly asks.
This way they encourage people to use 587 for submission. There were
quite some people on the net (swiss bloggers)
who critizized the way SC introduced traffic manipulation  and how they
communicated it (there was almost none). Some
doubted whether they countermeasure would really have impact since
people now are forced to use 587 for submission.
Swisscom got also quite critizied by other members of the swiss network
operators group (SwiNOG)

They are employing a Layer 4 redirection as far as they communicated.
