My company, Junction Networks (http://www.junctionnetworks.com) qualifies for the list.  Our facilities are located in the largest telco hotel in the country.  We of course notify our customers of any changes or maintenance windows when the arise.  We answer our email and answer the phone. We connect to multiple providers for service and constantly monitor call completion and quality and route calls accordingly.

got me thinking. I am interested in compiling a list
of "Business class" voip providers that have an
excellent reputation for:

#1) First and foremost most important for me: UPTIME.
#2) good colo with Tier 1 connectivity
#3) Sends email warning customers of system
configuration changes
#4) ALWAYS answers email within 1 hour
#5) ALWAYS answers phone calls to "tech support"

UPTIME is a tricky thing to predict.  But some basic questions about facility type and equipment would be great to have in chart form.  Along with affiliation lists with facility providers.  But it would be a lot of work to compile, many providers won't disclose the important details, and it would need to be updated constantly.
