On a Windows machine the answer is Live Communications Server 2005 and
Microsoft Office Communicator 2005.  Except for Skype, which is not sip or
iax.  Skype uses a peer to peer stack similar to Limewire and the other mp3
sharing protocols.  I do not believe there is a skype bridge for LCS...yet.

Sure, it is proprietary, Microsoft, probably a little expensive.  But that
should not stop you from considering it.
